中文摘要: 目的:医院感染监测系统是通过信息化的手段获取各项感染相关数据,从而对可能的感染进行预警的系统。本文分析医院感染监测系统对医院感染发生率的影响。方法:回顾性分析某三甲医院自2013年开始使用医院感染监测系统至2021年底各年度的医院感染监测数据,包括住院患者医院感染发生率、Ⅰ类手术切口感染率、器械相关感染率及多重耐药菌检出率。采用卡方检验比较数据间的差异。结果:2013年至2021年住院患者医院感染发生率由2.81%降至1.76%(P=0.000),Ⅰ类手术切口的感染率在2015年和2018年出现两次高峰,2018年之后逐年减低(P=0.000);导管相关血流感染发生率由2013年的2.89‰降低至2021年的0.00‰(P=0.001);呼吸机相关性肺炎发生率由3.61‰降低至0.88‰(P=0.020);导尿管相关尿路感染发生率由1.20‰降低至0.62‰(P=0.241);耐甲氧西林金黄色葡萄球菌的检出率逐年减低,由原来的50.41%下降至40.80%。结论:通过院感监测系统可有效降低医院感染的发生率,提高医疗质量和保障患者安全。
Abstract:Objective:Nosocomial infection surveillance system is a system that obtains various infection-related data by means of informatization,so as to give early warning of possible infection. Understand the impact of nosocomial infection surveillance systems on the incidence of nosocomial infections. Methods:A retrospective analysis of the nosocomial infection monitoring data of a certain grade A tertiary hospital from 2013 to the end of 2021,including the incidence of nosocomial infection in inpatients,the infection rate of Class I surgical incisions,the rate of device-related infection and multidrug resistance bacterial detection rate. The chi-square test was used to compare the differences between the data. Results:From 2013 to 2021,the incidence of nosocomial infection in hospitalized patients decreased from 2.81% to 1.76%(P=0.000). Infection rates for Class I surgical incisions peaked twice in 2015 and 2018. After 2018,it decreased year by year,and it was 0.20% in 2021,which was significantly different from 0.71% in 2018(P=0.000). The incidence of catheter-related bloodstream infection decreased from 2.89‰ in 2013 to 0.00‰ in 2021(P=0.001). The rate of ventilator-associated pneumonia decreased from 3.61‰ to 0.88‰(P=0.020). The incidence of catheter-related urinary tract infection decreased from 1.20‰ to 0.62‰(P=0.241). The detection rate of methicillin-resistant Staphylococcus aureus decreased year by year,from 50.41% to 40.80%. Conclusion:Nosocomial infection monitoring system can effectively reduce the incidence of nosocomial infection,improve medical quality and ensure patient safety.
