中文摘要: 目的:探讨江苏省全科医学专业住院医师规范化培训医师职业倦怠影响因素,为改善其职业倦怠提供依据。方法:2019年3月-4月,采用配额抽样从江苏省各地级市抽取1家国家级住院医师规范化培训基地,并用自制的全科住培医师职业倦怠影响因素问卷对基地内所有全科住培医师进行调查,共计13家基地682名全科住培医师,利用探索性因子分析和有序logistic回归分析研究其影响因素。结果:轻度职业倦怠检出者占27.3%,中度检出者占21.5%,重度检出者占7.5%;经探索性因子分析,全科住培医师职业倦怠影响因素问卷共有17个题项,隶属于4个公因子,累计贡献率为69.74%,分别为学科因素、社会因素、个人因素和职业特征;有序logistic回归分析结果显示,学科因素(OR=0.113,OR的95%CI:0.080~0.146)、社会因素(OR=0.139,OR的95%CI:0.096~0.182)和个人因素(OR=0.226,OR的95%CI:0.150~0.301)的消极程度越高,职业倦怠的程度越高。结论:全科住培医师职业倦怠检出率较高,学科因素、社会因素、个人因素是全科住培医师职业倦怠的影响因素。
Abstract:Objective:To explore the influencing factors of job burnout in the trainees of Standardized General Practice Residency Training(SGPRT)in Jiangsu province,so as to provide a basis for improving their job burnout. Methods:From March to April2019,using quota sampling method,a national SGPRT sites was selected from each city of Jiangsu Province and the whole sample of the trainees in the sites was involved in the study by using the self-made questionnaire of the influencing factors of job burnout in the trainees of SGPRT. A total of 682 general resident doctors from 13 bases were investigated. The influencing factors were studied by using exploratory factor analysis(EFA)and ordinal logistic regression. Results:Slight job burnout was detected in 27.3%,moderately detected in 21.5%,and severely detected in 7.5%. The questionnaire on the factors affecting the job burnout of the trainees of SGPRT in the whole subject were 17 items by EFA,with 4 common factors. The cumulative contribution rate was 69.74%. They are subject factor,social factor,personalfactor and occupational characteristic. The results of ordinallogistic regression analysis showed that the higher the negative degree of subject factors(OR=0.113,95%CI:0.080~0.146),social factors(OR=0.139,95%CI:0.096~0.182)and personal factors(OR=0.226,95%CI:0.150~0.301),the higher the degree of job burnout. Conclusions:The rate of job burnout among the GP trainees is high.Subject factor,social factor and personalfactor are the influencing factors of job burnout in the trainees of SGPRT.
