中文摘要: 目的：分析部分接收失能老人的医养结合机构服务提供现状、经验及困境。方法：通过调查问卷，了解江苏省20家接收失能老人的医养结合机构的性质、随访服务、特色养老和医疗相关服务等。结果：70%医养结合机构的性质为医养合作一体化；随访服务内容相对完善，健康养老服务存在缺失；为失能老人提供服务，离不开政策和政府财政的支持；面临着专业人才不足、缺乏激励机制等困境。结论：20家医养结合型养老机构在为失能老人提供专业医疗和养老服务的同时，应建立健康管理档案，开展重点随访服务，提供多层次服务，开通危急重症“绿色通道”。重视专业人才的培养，建立有效激励制度，全面提高失能老人的生活质量。
Abstract:Objective：To analyze the current status，experience and dilemmas of the services provided by medical and nursing institutions receiving some disabled elderly. Methods：Through the questionnaire，we know the nature，follow-up services，characteristic pension and medical services of 20 institutions which receive disabled elderly in Jiangsu Province. Results：70% of the combined medical and nursing institutions are integrated institutions. The follow-up service content is relatively complete，and there is a lack of health care services. Providing services for disabled elders is inseparable from policies and government financial support. They faced difficulties such as insufficient medical professionals，and lack of incentives. Conclusion：While providing professional medical and pension services for the disabled elderly，20 medical and pension institutions should establish health management files，carry out key follow-up services，provide multi-level services and open a “green channel” for critical and severe cases. Pay attention to the training of professional talents，establish an effective incentive system，and comprehensively improve the quality of life of the disabled elderly.
