中文摘要: 目的：探讨药师基于处方前置审核系统（prescription pre-review system，PPRS），在提高医院抗肿瘤药物超说明书使用审核实践中的作用。方法：采用回顾性分析方法，调取医院HIS系统2022年12月至2023年1月共2个月的超说明书使用抗肿瘤药物处方，由药师组成改进工作小组，调查抗肿瘤药物超说明书使用情况，查找问题原因，制定改进举措，并于2023年3月开始基于PPRS开展为期7个月的医嘱干预实践，比较对策实施前后抗肿瘤药物超说明书使用PPRS审核通过率。结果：药师干预抗肿瘤药物超说明书使用管理后，PPRS审核通过率由74.88%提高至93.62%，超说明书使用不合理例数占比由25.12%降低至7.98%。结论：药师基于PPRS参与抗肿瘤药物超说明书使用管理，可降低用药风险，保障临床用药的安全性、有效性、合理性。
Abstract:Objective：To explore the role of pharmacists in improving the hospital’s practice of reviewing anti-cancer drugs beyond the label using the prescription pre-review system（PPRS）. Methods：Through retrospective analysis，the prescriptions of anti-tumor drugs used off-label in the hospital’s HIS system from December 2022 to January 2023，a total of two months，were retrieved. An improvement working group composed of pharmacists was established to investigate the off-label use of anti-tumor drugs，find out the causes，formulate improvement measures. And starting from March 2023，a 7-month medical order intervention practice was carried out based on the PPRS，and the passing rates of PPRS audits for the off-label use of anti-tumor drugs before and after the implementation of the countermeasures were compared. Results：After the pharmacists intervened in the management of anti-cancer drugs prescribed beyond the label，the PPRS review approval rate was improved from 74.88% to 93.62%，and the proportion of unreasonable use of anti-cancer drugs was reduced from 25.12% to 7.98%. Conclusion：Pharmacists participating in the management of anti-cancer drugs prescribed beyond the label based on the PPRS can reduce the risk of medication and ensure the safety，effectiveness，and rationality of clinical medication.
