Abstract:Objective：This study aims to explore management strategies for reducing unplanned reoperations by analyzing the distribution of primary departments，reasons for reoperations，and patient demographics in unplanned reoperation cases at a certain grade A tertiary hospital in Guangxi from 2022 to 2024，thereby improving medical quality. Methods：This study selected 328 unplanned reoperation cases from January 1，2022 to December 31，2024，and analyzed the basic information，departmental distribution，incidence rate，causes，and outcomes of reoperation patients. Results：The results showed that the total incidence of unplanned reoperation from 2022 to 2024 was 2.46 ‰，with no statistically significant difference in incidence over three years（χ2=4.161，P > 0.05）. The top three causes of unplanned reoperation were postoperative bleeding（31.40%），postoperative hematoma（10.67%），and poor wound healing（9.45%）. The top three departments in terms of unplanned reoperation rates are cardiothoracic and vascular surgery，transplantation，and spinal surgery. Conclusion：Adding multidisciplinary discussions before the fourth level surgery，strengthening perioperative management，and implementing a surgical grading management system are important measures to ensure surgical quality and safety.
