中文摘要: 目的:探讨迷你临床演练评估量表(Mini Clinical Evaluation Exercise,Mini-CEX)在儿科住院医师规范化培训超声心动图教学中应用的可行性及效果。方法:对2021年1月-2022年12月在南京医科大学附属儿童医院超声科进行住院医师规范化培训的儿科专业的120名住院医师,超声心动图培训4周,2022年度学员为实验组,学习期间及出科考核中应用Mini-CEX结构式量表评估其临床能力,2021年度学员为对照组,采用传统教学及考核,两组学员出科时均进行理论+实践考试。评估Mini-CEX作为教学指导和考核方法的效果。结果:实验组学员学习期间及出科考核共4次使用Mini-CEX量表,60名住院医师共完成量表240份。所有学员出科时Mini-CEX检测结果均合格,医疗面谈、超声检查、临床判断、医患沟通、组织效能、人文关怀和整体评价的得分分别为8.7±0.5分、8.3±0.7分、8.5±0.6分、8.5±0.5分、8.4±0.6分、7.9±0.7分、7.7±0.6分。与对照组相比较,实验组学员出科理论成绩与实践成绩均较有不同程度的提高,差异具有统计学意义。结论:Mini -CEX 评估体系在儿科专业规培学员超声心动图教学中简便易行,操作性强,对规培医师的全方位培养有一定的帮助,可以作为儿科住院医师超声心动图教学的测评方法。
Abstract:Object:To explore the feasibility and application effect of Mini-CEXEvaluationin echocardiography teaching of residents in pediatrics standardized training. Method:One hundred and twenty residents who received standardized residence of pediatrics in the department of Ultrasonography in Children’s Hospital of Nanjing Medical University from January 2021 to December 2022 were selected as research objects. Echocardiography training for 4 weeks. The residents in 2022 were selected as the experimental group:the Mini-CEX evaluation was used to evaluate their clinical ability during the learning period and the examination. The residents in 2021 were selected as the control group:traditional teaching and assessment were used. All of them in both groups underwent theoretical and practical examinations when they left the department. To evaluate the effect of Mini-CEX as a teaching guidance and assessment method. Results:The Mini-CEX scale was used for 4 times during the study period and in the examination. In this study,a total of 60 residents participated in the evaluation and 240 cases were completed. The Mini-CEX test results of all residents were qualified. The scores of medical interview,ultrasound examination,clinical judgment,doctor-patient communication,organizational effectiveness,humanistic care and overall evaluation were 8.7±0.5,8.3±0.7,8.5±0.6,8.5±0.5,8.4±0.6,7.9±0.7,7.7±0.6,respectively. Compared with the control group in 2021,the theoretical and practical scores of the experimental group were improved to varying degrees compared with the control group,and the difference was statistically significant. Conclusion:The results have showed that the Mini-CEX scale is simple and feasible in the teaching of echocardiography for pediatric residents. It maybe helpful to improve the clinical ability of pediatrics residents and it can be used as an evaluation method for the teaching of echocardiography for pediatric residents.
