中文摘要: 目的:探索专科护士与社区护士组队模式在基层公共卫生服务中的实践效果。方法:成立由护理部主任-科护士长-护士长-驻点护理人员组成的核心管理小组;根据医联体内社区居民及患者的疾病特点及护理需求,结合医院专科护士的专业领域,确定心血管、糖尿病、伤口造口、康复、营养、静疗、老年7个类别的共7名专科护士作为人才资源,与社区21名社区护士形成“1+3”组队模式;制定专科护士+社区护士组队工作计划,确定工作时间、工作范畴和工作职责;制定组队培训大纲和工作方案。采用“3日工作模式”,第1日开展理论和技能培训;第2日预约病人到社区医院给予现场指导,社区护士作为辅助人员现场学习;第3日专科护士与社区护士组队开展上门指导;整个方案实施时间为6个月。结果:社区护士可开展的公共卫生服务项目得到扩展;社区护士专科能力自评在方案实施后有较大提高,差异有统计学意义(P<0.01)。结论:三甲医院专科护士是社区公共卫生服务建设的重要组成人员,在社区护士能力培养方面可以承担重要作用;社区也将成为专科护士职业发展的平台;应加大对社区专科护士的培养力度,为居民及病人培养健康管理人才。
Abstract:Objective:To explore the practical effect of the team model of specialist nurses and community nurses inbasic public health services. Methods:A core management team composed of the director of the nursing department-the head nurse-the head nurse-the on-site nursing staff was established. According to the disease characteristics and nursing needs of the community residents and patients in the medical treatment combination,combined with the distribution of specialized nurses in our hospital.It was determined a total of 7 specialist nurses in 7 categories of cardiovascular,diabetes,wound stoma,rehabilitation,nutrition,static therapy,and elderly are used as human resources,forming a “1+3” team model with 21 community nurses in the community. Formulating specialist nurses and community nurses set up a team work plan,determine their working hours and work scope,and clarify their job responsibilities,formulate team training outlines and work plans. “3-day work model”,theory and skills training will be carried out on the first day;on the second day,patients will be scheduled to the community hospital to give on-site guidance,and community nurses will serve as auxiliary staff to learn on-site;on the third day,specialist nurses and community nurses will team up for on-site guidance;The implementation time of the entire program is 6 months. Results:The public health service items that community nurses can carry out were expanded;the self-evaluation of community nurses’ specialized ability was greatly improved before and after the implementation of the program,and the difference was statistically significant(P < 0.01). Conclusion:The specialist nurses of grade A tertiary hospital are an important part of the construction of community public health services,and they can play an important role in the capacity training of community nurses. The community will also become a platform for the professional development of specialist nurses. The state should increase the training of community specialist nurses,to reserve talents for the health management of residents and patients.
