中文摘要: 目的:对一所综合性三甲医院的医务人员及患者的医患沟通现状进行研究,以期为提高医务人员的医患沟通效果和能力,改善患方的沟通满意度及构建和谐医患关系提供参考建议。方法:采用随机抽样方法,从该院随机抽取100名医务人员和100名患者进行问卷调查,对医患沟通的体会、信息、技巧、态度、影响因素等方面对医务人员的沟通效果和能力进行研究,比较医患双方对医患沟通的不同认识。结果:医患双方对医患沟通总体认知差异有统计学意义,医方更认可沟通的重要性(χ2=21.012,P=0.000);医患双方对医患沟通的内容选择上不一致,平等沟通、知情同意告知、沟通内涵等工作有待提高;医患双方对医患沟通的满意感受不一致,医方(68.9%)的满意度低于患方(88.4%);医生有业务繁忙、缺乏沟通技巧、认识不足等原因;而患者缺乏医疗常识、对医生期望过高、风险预估和信任不足及社会舆论导向是影响医患沟通效果的重要因素。结论:医方应从增加人文关怀内涵,规范知情同意告知,构建良好沟通环境,提升沟通技巧运用等方面着手,提升医务人员的沟通能力,构建和谐医患关系。

Abstract:Objective:To investigate the doctor-patient communication status quo between medical staff and patients in a first-class general hospital. It’s expected to enhance the doctor-patient communication effect and ability for medical staff,and improve the communication satisfaction of the patients as well as provide referent suggestions for the construction of harmonious relationship. Methods:By means of random sampling method,100 medical workers and 100 patients were randomly selected from the hospital to conduct a questionnaire survey. The experience,information,skills,attitudes,influencing factors and other aspects of communication between doctors and patients were studied to compare the different understanding between doctors and patients on doctor-patient communication. Results:There were significant differences in the overall cognition of doctor-patient communication between doctors and patients. The importance of doctor-patient communication was more recognized by doctors(χ2=21.012,P=0.000);the contents of doctor-patient communication between doctors and patients were inconsistent; the work of equal communication,informed consent informing and communication connotation needed to be improved;the satisfaction of doctors and patients with doctor-patient communication is inconsistency; the satisfaction of doctors(68.9%)was lower than that of patients(88.4%); the doctors are due to busy work,lack of communication skills and awareness,etc,and patients lack of medical knowledge,high expectations of doctors,lack of risk assessment and trust and public opinion guidance,which are important factors affecting the effectiveness of doctor-patient communication. Results:doctors should focus on the following aspects containing increasing the connotation of humanistic care,standardizing informed consent informing,building a good communication environment,enhancing the use of communication skills,improve the communication ability of medical staff,etc. So as to build a harmonious doctor-patient relationship.
