中文摘要: 目的:在国家传染病智能监测预警前置软件(以下简称“国家前置软件”)应用背景下,探讨三甲医院传染病医防融合监测预警的管理路径。方法:以心血管专科特色突出的首都医科大学附属北京安贞医院(以下简称“北京安贞医院”)为研究对象,采用案例分析法,深入解析其传染病医防融合监测预警管理体系的构建与实践路径。结果:该院构建了“国家前置软件+院内信息系统(hospital information system,HIS)监测”双轨并行的监测体系,以国家前置软件落实国家标准,以院内HIS监测实现个性化功能补充,并搭建“公共卫生科—临床科室自检员—临床医师”三级协同管理体系,形成“智能预警+线下督促”的全流程闭环管理路径。2025年12月,在该院感染科流行性感冒报告数较2024年同期增加124.25%的情况下,24 h内报告及时率提升至100%,临床医师预警响应时间缩短4.04 h,公卫医师预警响应时间缩短1.31 h,传染病防控质效显著提升。结论:双轨并行的监测模式突破了单一平台的应用局限,构建的三级协同管理体系具有较强的可复制性,可为同类医疗机构深化医防融合、提升传染病监测预警能力提供参考。
Abstract:Objective:Against the backdrop of implementing the National Infectious Disease Intelligent Surveillance and Early Warning Prepositioned Software(hereinafter ‘the National Pre-positioned Software’),this study aims to investigate the management pathway for integrating medical treatment and prevention in infectious disease surveillance and early warning in grade A tertiary hospitals. Methods:A case study was conducted at Beijing Anzhen Hospital,Capital Medical University(hereinafter ‘Beijing Anzhen Hospital’),a grade A tertiary hospital with a prominent cardiovascular specialty. The construction and implementation of its integrated infectious disease surveillance and early warning management system were analyzed. Results:The hospital established a dual-track parallel monitoring system comprising the National Pre-positioned Software and the hospital information system(HIS)surveillance. The National Pre-positioned Software was used to implement national standards,while HIS surveillance served to supplement personalized functions. A three-tier collaborative management system was also established,involving the Public Health Department,clinical departmental self-inspectors,and clinicians,forming a full-process closed-loop management pathway characterized by “intelligent early warning and offline prompting”. In December 2025,despite a 124.25% increase in the number of influenza cases reported by the Department of Infectious Diseases compared with the same period in 2024,the timely reporting rate within 24 hours increased to 100%,the response time for clinician alerts was reduced by 4.04 hours,and the response time for public health physician alerts was reduced by 1.31 hours,the quality and efficiency of infectious disease prevention and control significantly improved. Conclusion:The dual-track parallel surveillance model breaks through the application limitations of a single platform,and the three-tier collaborative management system demonstrates strong replicability. This provides a referable practical pathway for similar medical institutions to strengthen the integration of medical treatment and prevention and enhance their infectious disease surveillance and early warning capabilities.
