中文摘要: 目的:构建一种基于人工智能(artificial intelligence,AI)大语言模型(large language models,LLMs)技术、可用于医学教育的新型虚拟患者(virtual patient,VP)系统,探究该系统在外科住院医师培训中的应用效果。方法:选取2024 年9 月至10月在南京医科大学第一附属医院外科轮转的专业型硕士研究生为研究对象(N=20),随机分为试验组和对照组,分别同时进行为期1周的外科学临床问诊训练。试验组采用LLM 虚拟患者系统教学形式,对照组采用传统方式教学。课程结束后,两组进行为期2个月的临床轮转。轮转结束后对20名学员2个月的住院病历进行病历质量和患者满意度评估。结果:两组在年龄、性别、学历、摸底考试成绩等方面差异无统计学意义(P>0.05)。试验组同学住院病历质量的平均得分为86.2分,高于对照组的80.4分(P < 0.05)。在患者满意度方面,试验组同学84.5的平均分超过对照组的78分(P < 0.05)。结论:基于LLM 的VP 系统提升了学员对问诊以及入院记录书写的掌握程度,也促进了其临床思维能力的培养,该教学方法可为其他医学高等教育群体提供新的教学工具和思路。
Abstract:Objective:To construct a new virtual patient(VP)system based on artificial intelligence(AI)large language models(LLMs)technology,which can be used for medical education,and to explore the application effect of the system in standardized training for surgical residents. Method:Professional master’s students who rotated in the Department of Surgery at the First Affiliated Hospital of Nanjing Medical University from September 2024 to October 2024 were selected as the research subjects(N=20),randomly divided into an experimental group and a control group,and underwent a one week clinical consultation training in surgery simultaneously. The experimental group used LLM virtual patient system teaching format,while the control group used traditional infusion teaching. After the course,both groups underwent a 2-month clinical rotation. After the rotation,evaluate the quality of medical records and patient satisfaction of 20 students’ bed medical records within 2 months. Result:There was no statistically significant difference between the two groups in terms of age,gender,education,and test scores(P > 0.05). The average score of hospitalization medical record quality for the experimental group was 86.2 points,which was higher than the control group’s 80.4 points(P < 0.05). In terms of patient satisfaction,the experimental group also scored an average of 84.5 points higher than the control group’s 78 points(P < 0.05). Conclusion:The VP system based on LLM has improved students’ mastery of consultation and admission record writing,and also promoted the cultivation of their clinical thinking ability. This teaching method can provide new teaching tools and ideas for other medical higher education groups.
