中文摘要: 目的:核查某三甲医院甲状腺外科编码情况,分析编码错误原因,提高编码质量。方法:随机抽取某三甲医院2022年甲状腺外科病案首页数据共计405份,核查疾病和手术编码情况,统计编码错误类型,针对易错案例分析错误原因,指出编码的正确查找路径,探讨甲状腺疾病和手术分类的编码方法。结果:甲状腺外科405份病案中,疾病错误59份,错误率14.57%;手术错误71份,错误率17.53%。甲状腺疾病分类编码常见错误为甲状腺癌形态学编码错误、甲状腺多发结节错编为甲状腺结节、颈淋巴结转移编码错误、胸骨后甲状腺肿编码错误;甲状腺手术分类常见错误类型包括甲状腺切除范围有误、胸骨后甲状腺切除编码有误、腔镜甲状腺术式有误、颈部淋巴结清扫术编码有误等。结论:编码员应充分掌握甲状腺疾病与手术编码的分类规则,具备必要的临床知识,通过加强与临床沟通,规范临床书写,实现正确编码,也可借助信息化手段制定质控规则,提升编码质量。
Abstract:Objective:To inspect the coding status of the thyroid surgery department in a certain grade A tertiary hospital,analyze the causes of coding errors,and enhance the coding quality. Methods:A total of 405 data from the first pages of thyroid surgery medical records in 2022 of a certain grade A tertiary hospital were randomly selected to verify the disease and surgical coding,analyze the types of coding errors,explore the causes of errors in error-prone cases,indicate the correct search path for coding,and discussion the coding methods of thyroid diseases and surgical classifications. Results:A total of 405 cases of thyroid surgery were sampled,with 59 cases of disease errors and an error rate of 14.57%. There were 71 surgical errors,with an error rate of 17.53%. The Common errors in the classification of thyroid diseases are morphological coding errors for thyroid cancer,multiple thyroid nodules are miscoded as thyroid nodules,cervical lymph node metastasis is wrongly coded,and retrosternal goiter is coded incorrectly. The common errors in the classification of thyroid surgery included incorrect scope of thyroidectomy,incorrect coding of retrosternal thyroidectomy,incorrect endoscopic thyroidectomy,and incorrect coding of cervical lymph node dissection. Conclusion:Coders should fully grasp the classification rules of thyroid diseases and surgical coding,possess the necessary clinical knowledge,strengthen communication with clinical staff,standardize clinical writing,and achieve correct coding. Information technology can also be used to improve coding quality by establishing quality control rules.
