临床医学见习教学现状评估和提升策略探讨△

摘要:
目的:通过调查临床医学专业见习教学现状,探讨见习教学质量提升策略。方法:采取回顾性研究,通过文献检索、专家咨询等方法设计问卷,选取我院临床医学2017级-2020级87名见习同学进行匿名问卷调查,问卷内容由基本信息、倾向见习形式和见习评估量表组成,对见习期间担任理论授课、临床带教老师和教学管理老师的教学质量进行测评。用SPSS 26.0统计软件包进行数据分析,先对总量表和各维度分量表进行信度和效度分析,对于符合正态分布的计量资料用均数±标准差表示,采用t检验比较两组间的差异,分类资料用 N(%)表示,用χ2检验比较组间差异,定量资料的相关性用Person相关性分析,P < 0.05为差异具有统计学意义。结果:信效度分析表明量表具有较好可靠性和有效性,男生在教师教学专业维度、教师思政教学维度、教学管理效果维度评分均大于女生,差异具有统计学意义(P < 0.05),各维度条目平均分中教学思政维度得分最高,其次是教学管理效果维度,最低的是学生学习维度。学生学习维度和学生思想意识维度与教师专业教学、思政教学、教学管理效果均呈现正相关关系(P < 0.05),其中学生思想意识与教师教学专业相关性达到0.684(P < 0.05)。学生最倾向的见习方式依次是手术观摩、科室轮转、课堂讲课、小组讨论、模拟病人。结论:通过完善并落实见习医院教学制度,增强女生体质提高女生的见习教学质量,增加手术观摩次数,加强教师教学专业培训和教学质控考核,改进OBE教学理念、培养学生学习分析能力和临床思维,关注学生情感目标和价值目标建设,以此培养出更高质量的医学人才。

Evaluation of the Current Situation of Clinical Medical Clerkship Teaching and Discussion on Improvement Strategies△

Abstract:
Objective:Investigates the current situation of probationary teaching in clinical medicine and discusses the strategies for improving the quality of probationary teaching. Methods:A retrospective study was carried out to design a questionnaire through literature retrieval and expert consultation,and 87 interns of clinical medicine from 2017 to 2020 in our hospital were selected for an anonymous questionnaire survey. The questionnaire content consisted of basic information,orientation to internship form and internship evaluation scale. To evaluate the teaching quality of theoretical teaching,clinical teaching and teaching management teachers during the internship. SPSS 26.0 statistical software package was used for data analysis. First,the reliability and validity of the total volume table and each dimension subscale were analyzed. For measurement data conforming to normal distribution,the mean standard deviation was used to compare the differences between the two groups,the classification data was represented by N(%),and the differences between the groups were compared by χ2 test. The correlation between quantitative data and quantitative data was analyzed by Person correlation,and(P < 0.05)was considered statistically significant. Result:The reliability and validity analysis showed that the scale had good reliability and validity. The scores of The scores of male students in the dimensions of teacher teaching specialty,teacher ideological and political teaching and teaching management effect were all higher than that of female students,and the difference was statistically significant(P < 0.05). Among the average scores of each dimension,the score of teaching ideological and political teaching was the highest,followed by the dimension of teaching management effect,and the lowest was student learning. The dimensions of students’ learning and students’ ideological consciousness were positively correlated with the effect of teachers’ professional teaching,ideological and political teaching and teaching management(P < 0.05),and the correlation between students’ ideological consciousness and teachers’ teaching major reached 0.684(P < 0.05). The most recommended ways of internship were operation observation,department rotation,class lecture,group discussion,and simulated patient. Conclusion:Through improving and implementing the teaching system of trainee hospitals,enhancing girls’ physique and improving the quality of trainee teaching of girls,increasing the number of surgical visits,teachers’ teaching professional training and teaching quality control assessment,improving OBE teaching concept,cultivating students’ learning analysis ability and clinical thinking,and paying attention to the construction of students’ emotional goals and value goals,so as to cultivate medical talents of higher quality.
