中文摘要: 目的:分析苏州市院前急救卒中患者分布情况,调整资源布局,整体提升卒中救治能力。方法:以智慧急救平台为基础,从苏州市急救中心提取2018-2022年院前急救卒中患者病历资料,进行回顾性分析。结果:院前急救卒中患者男女比为1.67∶1,平均年龄为68.79±15.47岁;平均呼救-到院时间为38.34±17.28分钟;一年中院前急救卒中病例主要集中于冬季(10-12月),一天中的高峰时段为7-19时;主要发生区域在姑苏区、吴江区和吴中区,卒中病例既往病史分布最高的三项为高血压(58.06%)、脑部疾病(22.72%)和糖尿病(19.81%);FAST症状分布中肢体无力(68.82%)、言语不清(57.49%)、面瘫/口角歪斜(34.92%)在年龄分布方面存在显著差异。结论:院前急救卒中病例在人群、时间和区域的分布上存在一定的规律,相关部门可优化区域急救资源配置,提高院前医务人员的卒中救治水平,并针对高危因素的老龄人口进行卒中科普教育,以全面提升卒中病例的早期发现和早期救治能力。

Abstract:Objective:To analyze the distribution of pre-hospital emergency stroke in Suzhou,adjust the distribution of resources,and improve the overall ability of stroke treatment. Methods:based on the smart first aid platform,the medical records of stroke patients in pre-hospital emergency rescue in 2018-2022 were extracted from Suzhou Emergency Center and analyzed retrospectively. Results:the male to female ratio of pre-hospital emergency stroke patients was 1.67∶1,the average age was 68.79±15.47 years old,and the average call-to-hospital time was 38.34±17.28 minutes.Pre-hospital emergency stroke cases are mainly concentrated in the winter (October-December) of the year,the peak time of the day is from 7 to 19 o’clock,and the main occurrence areas are in Gusu District,Wujiang District,and Wuzhong District. The items included high blood pressure(58.06%),brain disease(22.72%),and diabetes(19.81%). In the FAST symptom distribution,limb weakness(68.82%),slurred speech(57.49%),facial paralysis/creeping mouth(34.92%)There are significant differences in age distribution. Conclusions:there are certain rules in the distribution of pre-hospital emergency stroke cases in the population,time,and region. Relevant departments can optimize the allocation of regional emergency resources,improve the stroke treatment level of pre-hospital medical staff,and conduct stroke science education for the elderly population with high-risk factors. comprehensively improve the early detection and early treatment capabilities of stroke cases.
