中文摘要: 目的:分析医院门诊退号情况及其原因,探讨改进门诊服务的有效策略。方法:收集某公立医院单个院区2019年1月-2022年6月的挂号退号数据,比较不同专科、挂号类别、就诊方式、就诊日期的退号率差异,通过问卷调查分析患者退号原因,并针对性实施门诊服务改进策略。结果:医院总体退号率逐年下降,从2019年的4.25%降至2022年的2.25%;周末的退号率(>4%)显著高于工作日(均<3.6%);总体退号率由高到低依次是外科(6.96%)、儿科(5.31%)、内科(3.95%)和妇产科(3.04%);退号原因以挂错号、重复挂号、患者个人原因、候诊时间过久等为主,经门诊改进措施后明显改善。结论:医院可以退号原因分析为抓手,回溯门诊服务的问题和不足,持续强化制度管理、完善诊疗流程、强化监管考核,营造更加优质便捷的门诊诊疗环境。

Abstract:Objective:To understand the situation and reasons of outpatient withdrawal,and explore the effective strategies for improving outpatient service. Methods:Collect the data of registration and withdrawal in a single district of a certain public hospital from January 2019 to June 2022,compare the differences of registration withdrawal rates among different specialties,registration types,treatment methods and dates. And we collect the data of the reasons for withdrawal through questionnaires,to monitor the changes in the composition of the reasons and the effectiveness of the improvement strategy of outpatient service. Results:The overall withdrawal rate decreased from 4.25% in 2019 to 2.25% in 2022. The withdrawal rate on weekends(>4%)was significantly higher than that on weekdays(all<3.6%). The withdrawal rate from high to low is surgery(6.96%),pediatrics(5.31%),internal medicine(3.95%)and obstetrics and gynecology(3.04%). The main reasons for withdrawal are wrong registration,repeated registration,personal reasons of patients,and long waiting time,which have been significantly improved after implementing improvement measures. Conclusion:Through the analysis of the reasons for the withdrawal,we can trace back the problems and deficiencies of the outpatient service,andcontinue to strengthen the system management,improve the diagnosis and treatment process,strengthen the supervision and assessment,to create a more high-quality and convenient outpatient environment.
