目的：基于省全民健康信息平台及低收入人口管理工作需求，开发健康扶贫信息管理系统，实现对低收入人口健康状况的动态监测和精细化管理。方法：充分利用信息化手段，强化平台在互联互通方面的作用，整合全省汇聚的健康医疗数据，应用B/S系统模式，开发供全省使用的健康扶贫管理系统。结果：系统简单易用，符合对低收入人口健康管理的需求，实现了对低收入人口健康状况的动态管理。结论：及时获取低收入人口健康数据，自动推送给基层管理医生，面向健康扶贫对象精准提供健康评估、用药指导、引导就医等“一对一”服务，推动健康扶贫各项政策措施落地落实。

Objective：Based on the provincial health information platform and the needs of low-income population management，the health poverty alleviation information management system is developed to realize the dynamic monitoring and fine management of low-income population. Methods：Make full use of information technology，strengthen the role of the platform in interconnection，integrate the health and medical data gathered in the whole province，apply the B/S system model，and develop a health poverty alleviation management system for the whole province. Results：The system is simple and easy to use，which meets the needs of low-income population health management，and realizes the dynamic management of low-income population health status. Conclusion：Timely obtain the health data of low-income population，automatically push it to the grass-roots management doctors，accurately provide “one-to-one” services such as health assessment，medication guidance and medical guidance for the health poverty alleviation objects，and promote the implementation of various policies and measures of health poverty alleviation.
