中文摘要: 目的:构建急诊专病绿色通道信息系统,探讨其在急诊专病患者救治和管理中的应用效果。方法:通过建立多学科合作的卒中、胸痛绿色通道团队,应用物联网定位技术构建急诊专病绿色通道信息系统,实现患者信息在时间和空间上的同步传输与管理。比较该系统应用前后脑卒中患者分诊处置时间、入院至CT完成时间(door to imaging,DTI)、门针时间(door to needle time,DNT)、护理记录平均所需时间、胸痛患者分诊处置时间、心电图完成时间及门球时间(door to balloon,DTB),以及患者抢救成功率、不良事件发生率和患者满意度的变化。结果:应用急诊专病绿色通道信息系统后,脑卒中患者分诊处置时间、DTI时间、DNT时间、护理记录平均所需时间明显缩短,差异有统计学意义(P<0.001);胸痛患者心电图完成时间以及DTB时间均明显缩短,差异均有统计学意义(P<0.001);患者抢救成功率由90.56%上升至97.85%,不良事件发生率下降,患者满意度显著提升。结论:基于物联网的急诊专病绿色通道信息系统能够缩短患者救治时间,提高急救效率,改善患者预后,保证患者的安全,有助于提升患者救治的全程质量与效率管理。
Abstract:Objective To construct the emergency special disease green channel information system and explore its application effect in the treatment and management of patients with acute special disease. Method Through the establishment of multidisciplinary cooperation stroke green channel team, the construction of emergency stroke green channel information system, the application of Internet of things positioning technology,so as to realize the synchronous transmission and management of information in time and space. The triage disposal time of stroke patients before and after the application of the system was compared, including the Door to imaging (DTI) time from admission to CT completion and the Door to needle time from admission to thrombolytic medication(DNT), the average time required for nursing records, the triage and disposal time of patients with chest pain, the completion time of ECG and door to balloon (DTB), as well as the changes in the success rate of patient rescue, the incidence of adverse events and patient satisfaction. Results After the application of emergency special disease green channel information system, the triage disposal time, DTI time, DNT time and the average time required for nursing records of stroke patients were significantly shortened, and the differences were statistically significant (P<0.001). The emergency stroke green channel information system based on the Internet of things can shorten the treatment time of patients, improve the efficiency of emergency treatment, improve the prognosis of patients, ensure the safety of patients, and help to improve the overall quality and efficiency management of stroke patients. The electrocardiogram completion time and DTB time of patients with chest pain were significantly shortened (P<0.001). The success rate of patient rescue increased from 90.56% to 97.85%, the incidence of adverse events decreased, and patient satisfaction increased significantly.
