中文摘要: 目的：研究苏北某三甲医院糖尿病共病费用支出项目及共病数与其费用支出相关性。方法：选择2020年1月1日-2020年12月31日苏北地区某三甲综合性医院内分泌科2型糖尿病出院患者893例进行研究，搜集患者共病数据及费用支出数据，观察比较患者不同类型费用支出及不同共病数量患者费用支出，分析糖尿病患者共病与其费用支出相关性。结果：在糖尿病共病患者各项费用中，花费最多的是药物费用，平均费用为3 476.69元，其次为检查费用，平均费用为2 453.17元，之后为治疗费用较高，平均费用为1 794.14元，护理费在花费中最少，平均为267.09元；不同共病数患者各项费用比较，差异均具有统计学意义（P < 0.05），且随着患者共病数量增加，患者治疗费用支出总体呈现升高趋势；不同共病类型患者间，住院总费用、治疗费、护理费、西药费、材料费、其他费用比较，差异具有统计学意义（P < 0.05），且心脑血管疾病、胃肠消化疾病及呼吸系统疾病患者住院总费用明显较高；共病数量与住院总费用、治疗费、护理费、检查费、药费、材料费显著相关（P < 0.05）。结论：苏北某三甲医院糖尿病共病患者医疗费用支出仍较高集中于药物费用，随着共病数增加，患者费用支出升高，且患者治疗费、护理费、检查费、药费、材料费支出与其共病数具有显著相关性。
Abstract:Objective：To study the correlation of comorbidity expenditure items and the number of comorbidities with the expenditure of diabetes patients in a certain grade A tertiary hospital in northern Jiangsu. Methods：A total of 893 discharged patients with type 2 diabetes from the endocrinology department of acertain grade A tertiary general hospital in northern Jiangsu from January 1，2020 to December 31，2020 were selected for study，and the data on comorbidities and expenses were collected and compared，and different types of patients were observed and compared. Expenses and expenses of patients with different numbers of comorbidities were analyzed，and the correlation between comorbidities and expenses of diabetic patients was analyzed. Results：Among the various expenses of patients with diabetes comorbidities，the most expensive is the cost of drugs，with an average cost of 3，476.69 yuan，followed by the examination cost，with an average cost of 2，453.17 yuan，followed by the higher treatment cost，with an average cost of 1，794.14 yuan. The nursing carecost was the least among the expenses，with an average of 267.09 yuan.The various expenses of patients with different comorbidities were gradually compared，and the differences were statistically significant（P < 0.05）. And with the increase in the number of patients comorbidities，the overall cost of treatment for patients shows an upward trend. Among patients with different types of comorbidities，there were statistically significant differences in the total hospitalization costs，treatment costs，nursing costs，western medicine costs，material costs，and other costs（P < 0.05）. And the total hospitalization cost of patients with cardiovascular and cerebrovascular diseases，gastrointestinal digestive diseases and respiratory system diseases is significantly higher. The number of comorbidities is significantly related to the total hospitalization expenses，treatment expenses，nursing expenses，examination expenses，medicine expenses，and material expenses（P < 0.05）. Conclusion：The medical expenses of comorbid patients with diabetes in a certain grade A tertiary hospital in northern Jiangsu are still relatively high and concentrated for the cost of drugs，with the increase of the number of comorbidities，the expenses of patients increased，and the expenses of treatment，nursing，examination，medicine，and materials were significantly correlated with the number of comorbidities.
