Abstract:Objective：To discover a new service model：the rehabilitation of cooperation model based on tertiary hospital and secondary medical rehabilitation institution on stroke patients. And evaluate the effectiveness of this model with ADL，patient satisfaction and discharge destination to solve the problem of insufficient capacity of secondary medical rehabilitation institutions，which leads to the rapid decline of stroke patients after they are discharged from tertiary hospitals. Method：90 stroke patients who were discharged from the rehabilitation department of our hospital from October 2019 to December 2020 but still need to continue to recover and admitted to secondary medical rehabilitation institutions were divided into two groups according to the wishes of the patients and their families. Patients who are willing to go to the second-level cooperative hospital for recovery are in the experimental group（46 cases in total，2 cases died of illness during the period），and patients who go to the second-level non-cooperative hospital are the control group（a total of 44 cases，1 case died of illness during the period）. The score of ADL and patients’ satisfaction（Patient self-evaluation，0-10 score，0-very dissatisfied，10-very satisfied），and discharge destination of patients（1go home and assume part of the role of the family；2health care institution）in the secondary hospital were used as the evaluation indexes of rehabilitation effect. Result：The score of ADL and satisfaction patients’ satisfaction of the experimental group is 44.32±10.38，the score of ADL and satisfaction patients’ satisfaction of the control group is 44.53±11.38，and there was no significant difference between the two groups（P=0.826）. The ADL score of the experimental group at 21 days of hospitalization is 74.55±15.73，the ADL score of the control group at 21 days of hospitalization is 49.19±12.10，there was significant difference between the two groups（P=0.000）. Rehabilitation satisfaction score of the experimental group at discharge is 9.66±0.78，rehabilitation satisfaction score of the control group at discharge is 7.51±1.08，there was significant difference between the two groups（P=0.000）. 38 people in the experimental group went home to assume part of the family role，6 people went to the health care institution，35 people in the control group went home to assume part of the family role，and 8 people went to the health care institution，here was no significant difference between the two groups（P=0.572）. Conclusion：Rehabilitation of cooperation mode based on Tertiary hospitaland and Secondary rehabilitation department can significantly improve the rehabilitation ability of Secondary rehabilitation department，and patients’ADL and degree of satisfaction.
