Abstract:Objective：To analyze the admission of insured trauma patients in the First People’s Hospital of Nantong in 2018，in order to provide the basis and suggestions for the health insurance management of such patients. Methods：The clinical data of 1 387 insured trauma patients admitted to the hospital in 2018 in the electronic medical record system were collected， and the epidemiological characteristics of patients were analyzed，such as time of injury，cause of injury and location of injury. Results：The proportion of insured trauma patients accounted for 2.84% of medicare patients，and the medicare payment was 44 million yuan，accounting for 5.4% of the total medicare payment. The per capita cost was 31，723 yuan，which was 1.9 times of the average payment of medicare patients. There were 51 people who did not meet the health insurance payment requirements，involving a cost of 2.299 5 million yuan. The ratio of male to female was 0.89∶1. The incidence of female patients was higher than that of male patients. Patients over 50 years old accounted for 76.57%. The ratio of urban to rural residents was 1.06∶1.7 4.07% of patients were able to seek medical treatment within 2 days after the accident. The average hospitalization days were 12.48 ± 9.87 days. Highest incidence was in August and September（9.66%）. 1 254 cases（90.41%）were accidental falls，admitted to the department of orthopedics，spinal surgery，neurosurgery，hand surgery and burn plastic surgery. Conclusion：the majority of the insured trauma patients in Nantong area are middle-aged and elderly. The main cause of injury is accidental fall，and the injured parts are limb injury，spinal injury and craniocerebral injury. The treatment cost is relatively high. Suggestions：① The hospital needs to do a good job in the payment approval work of insured trauma patients to prevent unqualified patients from entering the health insurance payment list and occupying medical insurance funds. ②The medical insurance administration department and the associated hospital should publicize the medical knowledge related to preventing falls and injuries at the grass-roots level，especially popularize the medical knowledge related to preventing accidental falls and the relevant preventive measures to the elderly population. ③ The medical insurance administration departments should publicize the concept of seeking medical treatment in time after accidental falls and injuries. It is suggested to improve the diagnosis and treatment capacity of community medical institutions，and be able to timely assess patients’ conditions and handle or refer them.
