中文摘要: 目的:探讨采供血机构丙氨酸氨基转移酶(ALT)测量不确定度分析前变异分量。方法:依据《献血者健康检查要求(GB18467-2011)》,随机选择240名符合条件的多次无偿自愿献血者作为研究对象,分别采用采集到分离不同时间间隔(0 h、8 h)、不同储存条件(常温和2~8℃)、不同离心温度(常温离心和4℃)、抗凝与促凝管及是否分离后储存(2~8℃)五种方式进行处理,采用SPSS 20.0统计软件进行方差分析。对有统计学意义的分量正确判断比重,用不可忽略的分量计算出分析前不确定度。结果:不同试管类型P > 0.05,说明不同试管类型对结果的影响没有统计学意义;采集到分离时间间隔不同,离心温度不同、储存条件不同及是否分离后储存P < 0.05,有统计学差异;采集到分离时间间隔和是否离心后储存两个分量的不确定度略小于重复性不确定度;离心温度和储存温度均大于重复性带来的不确定度。结论:标本采集到分离时间间隔、离心温度、储存温度和是否离心后储存的处理方式存在不确定度,在分析前不确定度评定时不可忽略。
Abstract:Objective:To investigate the variation components of the uncertainty of alanine aminotransferas(ALT)measurement before the analysis in blood services laboratory. Methods:According to the requirement of the physical examination of blood donors(GB18467-2011),a total of 240 voluntary blood donors eligible for multiple free voluntary donation were randomly selected as study object. The blood samples were treated at different separation interval,at different storage condition,at different centrifugation temperatures,at different collection tubes(anticoagulant tube v.s. coagulant tube),and underwent centrifugation after storage or before storage. The variance of each group and undifferentiated group was statistically analyzed by SPSS 20.0 software. For statistically significant components,the uncertainty before the analysis can be calculated by the non-negligible component. Results:The results of ANOVA showed that there were statistical differences between the groups coping with different sequence of centrifugation and storage,between the goups at different storage temperature,with different time intervals between sample collection and separation,and at different centrifugation temperatures(P < 0.05). Only the groups using different collection tubes showed no statistical difference(P > 0.05). The results indicated that the sequence of centrifugation and storage,storage temperature,time intervals and centrifugation temperature could influence the uncertainty of ALT measurement. Conclusions:The pre-analysis components,such as sequence of centrifugation and storage,storage temperature,time intervals and centrifugation temperature,should be strictly controlled . The uncertainty assessment before the analysis is not negligible.
