南京市雨花台区结合实际积极探索，开展服务模式改革试点工作。通过推行“个性化签约服务包”，针对不同重点人群设计不同内容的个性化签约，提高家庭医生服务的精准性；各社区借助不同区域优势推出个性化服务，区域间形成良性竞争；同时积极引进第三方参与模式，如引进互联网技术平台等，为居民提供更加丰富便捷的服务。该区在家庭医生个性化签约服务上取得了一定进展，积累一定实践经验，但服务能力仍有待提高，本研究提出了进一步发展的建议，使居民能够获得更加便捷高效的医疗卫生服务。

Through the implementation of “personalized contract service package”，Yuhuatai District of Nanjing City designed personalized contracts for different key groups to improve the accuracy of family doctor services. Each community launched personalized services using different regional advantages，and formed a healthy competition among regions. Third-party participation models，such as the introduction of Internet technology platforms，provided more rich and convenient services for the residents. The district has made certain progress in the personalized signing service for family doctors，and has accumulated some practical experience，but the service capacity still needs to be improved. This paper put forward further development suggestions for Yuhuatai District to improve the insufficient services，to provde more convenient. Efficient health care for the residents.
