Abstract:Objective：To explore the construction of a three-in-one health management　model for stroke patients based on intelligent information，and to evaluate the application effect. Methods：The patients who were hospitalized in our department of neurology from January 2015 to December 2016 and were discharged from the hospital in stable condition were selected. They were divided into two groups according to the scale of towns and streets，which were set as the control group and the intervention group，with 224 cases of cerebral infarction in the control group and 220 cases of cerebral infarction in the intervention group. The control group received routine health management from the township hospital after discharge；The intervention group received the three-in-one health management of stroke based on intelligent information. The levels of blood pressure，fasting blood glucose，cholesterol，triglycerides， daily living assessment（ADL）， mastery of stroke knowledge，and stroke recurrence rate within 18 months were evaluated before and 18 months after the management. Results：After management，the differences in blood pressure，fasting blood glucose，cholesterol，triglyceride，ADL and mastery of stroke knowledge between the two groups were statistically significant（P < 0.05 or Italic < 0.01）；There was a statistically significant difference in the recurrence rate of cerebral infarction between the two groups in 18 months（Italic < 0.01）. Conclusions：The three-in-one health management model for stroke patients based on intelligent information can effectively control the high-risk factors in stroke patients in primary health care institutions, reduce the complications and improve the quality of life of the patients.
