Abstract:Objective：To discuss the therapeutic efficacy and practical value of laparoscopic cholecystectomy（LC）and minilaparotomy cholecystectomy（MC），and to provide a reference for clinicians to choose an appropriate operational method. Methods：A retrospective study is used to select 1 426 cases of gallstone patients from January 2012 to December 2016. According to the different treatment methods，449 cases are in group MC and 997 cases are in group LC. The general situations before operations，the operation conditions，postoperative recoveries，complications，hospitalization fees，and the drug costs are compared in the two groups. Results：During the 5 years from 2012 to 2016，the number of LC operations increased year by year while the number of MC operations decreased year by year. There is no significant difference in the demographic characteristics and symptoms between the two groups，but the blood loss, postoperative ventilation time and the hospitalization time in LC group were obviously superior to that of MC group（P < 0.05）. Logistic regression analysis shows that age，sex，occupation，education，stone and admission pain had no significant effect on the postoperative outcomes. Conclusion：Laparoscopic cholecystectomy has a better therapeutic efficacy and quicker recovery，and is worth popularizing. However，if there is a risk of transferring to the open abdomen or the physicians are not skilled，the minilaparotomy cholecystectomy should be selected as appropriate.
