该文通过江苏省老年医院推进医疗资源对养老的介入与融合，实现医养融合发展“五个一”目标，即创建一个分院，全面运营管理一个护理院，开创一种“城乡联动”养老模式，开发一套医养结合慢病与健康管理软件，发起成立一个医养协作联合体的实践，表明医养融合是我国老龄化社会养老发展的必然趋势，是实现医疗、养老机构优势互补的有效途径，并指出医养融合需要更多的政策支持。

This paper takes the Jiangsu Geriatric Hospital as an example，which carries out the innovative practice of combination of medical and elderly careing，and achieves five goals，including founding a branch hospital，operating and managing a nursing home，creatingan urban-rural elderly care pattern，developing a medical and elderly care software which combining of chronic disease and health management，initiating a cooperation of medical and nursing. This practice indicates that，the combination of medical and elderly care is an inevitable trend of the development of old-age care，and also an effective way to realize the complementary advantages of medical and pension institutions. The paper also points out that more policy support is required for the combination of medical and elderly care.
