Abstract:This paper takes the Jiangsu Geriatric Hospital as an example，which carries out the innovative practice of combination of medical and elderly careing，and achieves five goals，including founding a branch hospital，operating and managing a nursing home，creatingan urban-rural elderly care pattern，developing a medical and elderly care software which combining of chronic disease and health management，initiating a cooperation of medical and nursing. This practice indicates that，the combination of medical and elderly care is an inevitable trend of the development of old-age care，and also an effective way to realize the complementary advantages of medical and pension institutions. The paper also points out that more policy support is required for the combination of medical and elderly care.
