株洲市2005—2018年学校突发公共卫生事件流行病学分析

摘要:
目的:了解株洲市学校突发公共卫生事件的流行病学特征,为科学制定防控措施提供依据。方法:收集株洲市2005—2018年报告的学校突发公共卫生事件资料,应用描述性流行病学方法分析。结果:株洲市2005—2018年共报告117起学校突发公共卫生事件,报告事件以一般级别(占79.49%)和未分级事件(占20.51%)为主,无重大和特别重大事件报告;事件类型以传染病疫情为主,共111起(92.26%),事件类别以呼吸道传染病事件为主(78 起,占66.67%),其次是肠道传染病事件(30 起,占25.64%);2017年和2018年学校突发公共卫生事件报告大幅上升,两年共报告学校突发公共卫生事件58起,占49.57%,每年事件的发生高峰为4-6月(48起,占41.03%)和11-12月(40起,占34.19%);报告事件数最多是茶陵县,25起(21.40%),高发学校为小学(69起,占58.97%)。结论:学校尤其是小学是突发公共卫生事件发生的重点场所,传染病疫情是学校突发公共卫生事件防控的重点,教育和卫生健康部门应加大对学校卫生的投入,健全联防联控机制,学校需加强晨午检、因病缺勤登记和免疫规划等综合防控措施。

Abstract:
Objective:To investigate the epidemiological characteristics of emergency public health events at schools in Zhuzhou City so as to provide a basis for scientifically formulating prevention and controld strategies. Methods:Descriptive epidemiological methods were used to analyze the data regarding emergency public health events at schools collected from Emergency Public Reporting System in Zhuzhou City from 2005 to 2018. Results:A total of 117 emergency public health events at schools were reported in Zhuzhou City during 2005-2018. Most incidents were at moderate level(79.49%) or ungraded level(20.51%),without severe incidents and special severe incidents. A total of 111(92.26%)emergencies were infectious disease events,and the main incident category was respiratory infectious diseases(78,66.67%),followed by intestinal infectious diseases(30,25.64%). Reports of public health emergencies at schools increased significantly in 2017 and 2018. In the past two years,58 public health emergencies were reported,accounting for 49.57%. The incidence peaks in each year were from April to June(48,41.03%)and from November to December(40,34.19%).The majority of the reported events occurred in Chaling county(25,21.40%). Primary schools were the high risk areas(69,58.97%). Conclusions:Schools are the areas with high incidence of public health emergencies,especially primary school. More attention should be paid to the epidemics of infectious diseases for preventing and controlling emergency public health events at schools. Education as well as health and wellness department should enhance the investment in school health,improve the mechanism of collaboration and intensify the comprehensive prevention and control measures,such as morning and noon inspection,sickness absence registration and immunization program.
