| 目的：了解昆山市医疗卫生机构人员对分级诊疗现状的认知，为进一步完善分级诊疗提供科学依据。方法：采用整群随机抽样方法，对昆山市二级医院和社区卫生服务中心医护人员进行问卷调查。结果：在分级诊疗认知程度、分级诊疗赞同情况、分级诊疗可行情况、分级诊疗解决就医难、转诊人数及转诊便利性等分级诊疗基本认知方面，医院和社区卫生服务机构人员的认知差异没有统计学意义（P > 0.05）。而在分级诊疗实施对诸如医院提供预约专家、专家下社区指导、开展业务讲座、社区人员收入降低、人才流失等，社区机构与医院调查对象认知分布差异有统计学意义（P < 0.05）。结论：医院和社区机构对分级诊疗总体认知基本一致，而在一些涉及人员因素的具体指标中，医院与社区调查对象存在明显差异。因此，要进一步夯实分级诊疗落地的具体措施，以便协调平衡推进分级诊疗实施。 |

| Analysis of the cognition status of hierarchical medical system in the medical staffs in Kunshan city△ |

| Objective：To analyze the cognition status of hierarchical medical system in the medical staffs in Kunshan city，and to provide evidence for further improvement of hierarchical medical system. Methods：Questionnaire survey was conducted among the medical staffs of the secondary grade hospitals and community health service center in Kunshan city by stratified cluster random sampling method. Results：There were no significant differences in the cognition degree，approval，feasibility of hierarchical medical system and the ability in solving difficult medical problems byhierarchical medical and number of referral patients between the staffs in the hospital and those in community health service center（P > 0.05）. While there were significant differences in the cognition of appointment experts in the hospitals，experts entering the community，and medical staffs’ income drop，talent loss and low level of community doctors（P < 0.05）. Conclusions：Since the implementation of hierarchical medical system in Kunshan City，there are obvious differences in the perception of hierarchical medical system between the relevant personnel of hospitals and community institutions. Therefore，it is important to further strengthen the propaganda of hierarchical medical system in order to effectively promote the implementation of hierarchical medical system. |
